Teacher Card Request

Teachers in the Sachem Central School District may use this form to request a Teacher library card for Sachem Public Library.

**NOTE**
Teacher cards are delivered to your school via the Sachem Pony Express.
Teacher cards are valid for all library materials and services, except study rooms, program registrations and museum passes.
Teacher cards expire after one year, but are renewable. Contact Patron Services to renew any expired card.
